
Just heard from Digby again — Jane's oncological surgeon came out to give an update and seemed cautiously optimistic without actually committing to anything until the final pathology reports are in (you know the way that doctors do that update without actually giving you any solid answer but only a feeling, right?). We've decided to go with the cautiously optimistic thoughts, because frankly that sounds awfully good about now. The surgeon went back in to finish the work and then there was to be some further testing from there, but things appear, at least at this point, to be proceeding as well as we could hope under the circumstances. Will update when I know something, in the meantime consider this an open thread.

I remember my first mammogram. What a drag. They brought me back TWICE because they thought they saw something. There were about ready to do a sonogram when they finally cleared me. After the third mammogram on my right breast I told the technician that if my right breast was permanently bigger than my left one it was all their fault.
Jane rocks, and I will get a mammogram.
This is the best place.
Oh good. This is the day! Jane gets the tumor taken out clean and will come back strong. Today my oncologist examined my throat and proclaimed me clear of cancer as far as this level of exam can show. It’s a wonderful great day. Love love love, this community is so full of love. Ahh…

I understand their caution, but if they’re even hinting at a positive outcome then things are most likely going as expected. That’s all you can ask of invasive surgery.

Thanks for the update, Christy. I’ve been thinking about Jane all day. I sit in meetings and look at the clock and think, “Jane must be in surgery now.”, “Jane’s doctor is probably looking at lymph nodes right now.” or “I wonder how Jane will like her new boobs?” (not to be insensitive, but I wonder about such things).
Jane’s news the other day really affected me in an unexpected way. It feels like it could be happening to my sister tho’ I have only been within 3 feet of Jane and never spoke a word to her. She doesn’t know me from adam but I feel like I know her. And I worry and hope.

My experience of this is that final pathology, frozen sectioning with screening for markers, etc, takes on average 48 hrs. to get the report. If more specialized markers/immunohistochemistry are desired, that info. could take longer.

You do that thing, Alison. Especially if you have a family history of breast cancer. My paternal grandmother and two of my father’s sisters died of breast cancer. His third sister was a breast cancer survivor.
If it’s detected early, it’s survivable. So get a mammogram.

Doctors seldom do cautiously optimistic unless they are very optimistic. Just have to make absolutely certain not to be confounded by some unexpected, or unseen complication. Cautiously optimistic is exactly what we want to hear, thank you Jane’s Docs. We shall come back for further updates, and hope for a great prognosis. What a tough cookie Jane is.
Thanks for the update and the marvelous Melissa Etheridge video. As a longtime married guy who sweated through a few (thankfully neg) breast biopsies of my wife, I think I have a (small) idea of what it all really feels like.
… Let me put in a quick word to the guys in the comments section here: Not to forget your periodic prostate exams. I was diagnosed ~13 months ago, had a full course of rad and am said to be OK now. But PC is (usually) not as serious a threat to guys as BC is to women.
You have so many friends on the ‘Net Jane, I hope these comments give you an idea of how much love there is out here for you. Waiting and hoping for best news about one of my heroes.

What Christy says above about scheduling your annual mammogram (or getting your wife/sister/girlfriend/mother to do so) and then some!!!
If you have a family history, get your regular doctor or OB/GYN to recommend the best breast surgeon/oncologist in your area and make an appointment to meet them and have them review your films in addition to the radiologist. Once you’re a regular patient, you already have a relationship with the doctor if anything shows up that needs treating. You have no idea how reassuring this is if you’ve spent even one night worrying about cancer. Plus mine gave me a lot of good tips for getting through the *change* comfortably.
When you leave from your mammogram, schedule your appointment for the next year … Greenwich (CT) Hospital rocks on this and won’t let you out the door without doing it.
I know, I know, I’m a fanatic on this one.

The initial report is based on sentinel node testing and fresh sectioning. A good initial report is cool, but of course the final report is what one uses to base further planning and treatment.
Cautiously optimistic is what we were the day and evening of the primary in CT. I’ll go with that.
Getting my mom who lives near the hospital to bring Jane some excellent chocolate in a couple of days.

Sounds bad , but this is EXACTLY the kind of clinic you want to go to that keeps bringing you back if any doubt, that even hounds you if that is what it takes. My partner and her sisters and mom schedule theirs every year at the same time and do lunch and almost every time one of them has been called back for more pictures. Which is really a pain and insurance doesn’t usually cover it. That is what we thought had happpened last year, but Northwestern Memorial in Chicago caught a very earlyl “.3 centimeter very agressive” so early you couldn’t see it, feel it, and the presurgery had to put in wires so they knew where to cut. Jan is doing fine. My mom is also a survivor now of 36 years.
Early detection, you can’t feel em if it is early.
And boyz don’t forget men get breast cancer too, but more importantly roll those stones on a regular basis.

Court is done for today.
We now stand at 30 qualified jurors on the pool, but we need 36. It took us all day to get 6, so the Judge adjourned for the night and hopes to complete jury selection Monday. Opening statements, he said, will not commence until Tuesday at the earliest.
The calendar is too full for any jury selection tomorrow though there will be a conference among attorneys tomorrow at 3PM EST, which, according to Jeffress, will cover small issues related to jury instructions, among other things unspecified.
I’m going to leave court now and will have an update and nightly recap later on.
Forgive me if this has been discussed before but I think we need to talk about some of the politics of breast cancer. While a diagnosis of breast cancer is a personal tragedy, there is a larger issue in that something (or things) is causing the breast cancer rate to increase at a terrifying pace. Most likely environmental factors.
While Jane needs to stay positive and fight her cancer, the rest of us should be angry. Too many women have this dreaded disease. The large cancer organizations emphasize detection and treatment (many are funded by corporations that make money from the machines and medicines), however less emphasis is put on prevention which would require changes in how we do business. For example, Avon, which is one of the leaders of the October breast cancer events, will not take parabens out of their cosmetics (parabens are suspected of being a possible breast cancer contributor).
I give my money to Breast Cancer Action http://www.bcaction.org/ and the Breast Cancer Fund http://www.breastcancerfund.org/–both are concerned about the environmental aspects of breast cancer. I encourage everyone to check them out.
As a breast cancer survivor of 3 years I am angry to see so many new members of our special sisterhood. I second the call to get mammograms and take care of ourselves individually but also think we need to start asking why there are so many of us getting sick.

All those having dental work done. Consider having crowns done in composit rather than any metal. There is some evidence that composite designed to go with your body chemistry works best. I had all amalgam fillings removed and a ton of caps done a year and a half ago. The composit adheres to the tooth leaving no space for decay to develop behind. There is quite a controversy about the whole issue.
This was on Balloon Juice today on a column about ….NEW HORIZONS IN THE CANCER FIGHT….. “This post is dedicated to Jane Hamsher, who needs to get better so that she can go on ticking off the John Coles of the apostate right.” Jane is certainly loved and would I’m sure appreciate this.

Amato will probably tell you he’s singing again, having a live Midnight Ramble at his studio in Woodstock twice a month, where guests like Emmylou show up unannounced. (i saw him drumming in SF for his daughter, and we spoke after the show for several hours, he was really horse.)
And now he’s singing again. As if Jane needed a role model.

“The biggest drop occurred in colorectal cancer with 1,120 fewer men and 1,094 fewer women dying of the disease….
Similarly, 666 fewer women died of breast cancer and 522 fewer men died of prostate cancer.”

Interesting. I’d read recently that breast cancer rates had dropped significantly in Canada and US (argues against govt anyhting), suggestion was that it reflected decreased use of hormone therapy for menopause. But that wouldn’t cover colorectal or prostate. Hmm, I have a client who is an epidemiologist specializing in cancer, I’ll see if she can give me a source for info.

(Given my reposting of the “be kind to the mods” comment on several of the threads in the last couple of days, I’m hardly one to intimate anything about posting something more than once. Hmmmm . . . something about a log in my eye comes to mind . . . )
The research has never been done, but back in the 1970’s my Cancer Surgeon advised me not to use any sort of under-arm deoderant that was an antiperspirant. Sweating, is indeed a waste disposal natural form, but antiperspirant closes down the pores and this function. It makes sense — soap and water work fine and you will never miss the product. Any bath soap that has an anti-bacterial component will eliminate odor, and generally protect till the next bath. As I said, there is no authoritative research on this, no experimental data — but it just makes sense if you think about it, and it is a low cost prevention just in case he was guessing right.

The antibacterial aspect of soaps is itself problematic: there’s evidence it stays in the water past the treatment plants, and it can lead to resistance in bacteria (always evolving, those guys).
Also, I’d only change out a filling if it was going to be damaging the tooth it was in. (There isn’t much evidence, AFAIK, for mercury leaching from amalgam.)

Last time I saw him was on a Mississippi riverboat midnight concert at the King Biscuit Blues Festival. His voice was in rough shape but I must say the soul of that night was incredible.

I had lots of fillings so did many caps and repairs. The most I’ll ever need again is a cleaning or some minor stuff. My dentist recycles the mercury. She studied with the controversial Dr. Huggins.

Waccamaw – my dentist also said that in 20 years of such work she had only taken out a very few fillings that didn’t have decay behind them since the amalgam fillings just sit in the tooth and thus provide a place for the decay to take hold. The composit bonds to the tooth eliminating the possibility.

Thanks for the update. As with all, my thoughts are with Jane for a positive outcome and a speedy recovery. I can not express just how much admiration and gratitude I feel towards her, and I’m certain I am not alone.
I have been a patient in St John’s, years ago when I broke my leg in a bicycle accident, and found the facility and the staff first rate. I am confident she is in competent hands.
Also, since I live fairly close by, if there is anything Jane, or Digby, needs, like a home cooked meal or whatever, get in touch with me and I will be honored to do whatever I can. I’d offer to visit but often patients prefer not too many distractions and commotion. Let me know.
OK, I stopped reading the comments a 100. Some of the links provided by commenters might have mentioned it but if you females aren’t doing breast self examination, you’re inviting trouble. Breast self examination is a very effective means of catching cancer early. The AMA recommendations for doing it might be adequate but there are better methods. Check out Mammacare (with which I am not affiliated but it was researched by one of my former professors). Same with testicular examination for (generally) younger men. In either case, get your partner to help! It can be fun!

I would just like to add this: yes, get a mammogram, your best chance is to find a place with newer machines and ask for a digital mammogram – they are better for younger(denser) breasts, they also provide fewer false positives.
Know too that mammogram is only one piece of the diagnostic tools and are not anywhere close to failsafe. DO yearly clinical exams and monthly personal exams – my Dad (a doctor) said most of the women he treated found their lumps.
Get any lump checked out!
My sister is going through chemo with stage II cancer right now and will be getting surgery.
She had a lump a year ago and the mammogram came back “clean”. But there is something inexplicable about what happened with her – I’m baffled that her doctor didn’t do a follow up or order a biopsy. But she may have refused due to her fundamentalist religion too…
Mammograms cannot see all cancer. They are best for identifying it – but are by no means a “clean bill of health” if they don’t see it. Not to panic – just know you need to understand it and do for yourself the additional steps you can.
You probably know this, but I’ll say it anyway. The NIH Clinical Center in Bethesda, MD and their contract sites throughout the U.S. have ongoing protocols which are well worth investigating, some of which can be done online…

from experience, “cautiously optimistic thoughts” is not the way to go. You’re just starting out and the road could get long and bumpy. Healthcare workers will give you false “feelings” all the time so as to avoid your possible adverse reactions.
Up and down with cautiously optimistic alternated with ‘we’ve had a minor setback’ zaps all your energy that you may need for more important things that could happen. “Hoping for the best, while preparing for the worst” will leave you better prepared if things don’t go as well as hoped.

As I said — no studies of the links with long term use of antiperspirants and cancer have been commissioned. And since antiperspirants are a billion dollar industry, don’t expect good controlled research soon. But we live in a culture where fear of BO has been on offer at least since the 1920’s, (can you sing a Lifeboy advertising song?) and yes indeed the incidence of Breast Cancer climbs as the adoption of the antiperspirant mode of preventing one sort of BO also climbs. Male antiperspirants were introduced much later than female ones — and the incidence of male breast cancer seems to track the wide adoption of the product. The point is pretty simple, you can avoid BO and do so without using these products. They may be harmless — but they may not be.
And yes, I understand the argument about putting anti-bacterial soap into wastewater — but I suspect by the time the sewage reaches the processing plant, de little bugs are pretty well worn out dealing with the city sewer system. Someday we will understand the balance between proper exposure to bugs so as to enhance immune protection, and when to protect against odd bugs given weakened immune systems. As a Cancer survivor who messed with my immune system with 4 courses of Chemotherapy, anti-bacterial soap makes sense. If I had a healthy small child, I think I might weigh in on the side of some exposure so as to enhance natural lifetime immunity to this and that. People who are doing, or have done Chemotherapy need to avoid odd sinks of strange bacteria and other bugs. For instance, one wants to avoid spending time with small children who go to nursery school who have the sniffles or some other minor and unremarkable illness. You should also avoid hospitals and nursing homes.
My Chemotherapy had a very strange side-effect. From the time I was a kid up till when I was diagnosed with cancer at age 31, I had at least one strep infection every winter. Sometimes two. I had another between surgery and the beginning of Chemotherapy. Since March 17th of 1970 when my chemo began, I have not had a Strep infection. 37 years without a Strep Throat!!! I do not recommend this method of avoiding the infection. About ten years after the chemo treatment, I remarked on this odd fact to my chemotherapist’s lab and research assistant, and we actually did a culture, and I am apparently immune to strep.
These are just basic comprehensions that many who take this trip come to deal with and understand — all this “new medicine” and the science it is based in are experimental, and the researchers and the doc’s can only do and know so much — the variables that might be really important in preventing re-occurance or even prevention in the first instance are so numerous that only the most obvious can be studied. My Chemotherapy, for instance, was a product of Nixon’s War on Cancer, and I am in the second phase study, just after the first screening phase on stage 4 and 5 patients — the drug was dropped after four years because more effective Melanoma ones came along, In fact after ten years I was the only survivor in my test group, thus really no way to consider whether immunity to strep might be more than just a single case phenonema or what it might mean. Believe me, this is a hell of a life trip.
What we know about Melanoma — it is probably genetic and enhanced by the sun. If you are Scots, Irish, Bylorussian, Welch, Austrian or French Celtic, you probably have a genetic predisposition to it. If you are blond, red haired, or blue-green eyed, you may be predisposed. If so, you should totally distain the sun, particularly the culture of the sun-deck and the swimming pool, the beach or even the ski culture unless you protect yourself. (one of the most horrible evenings I had on Cancer Ward was with a friend who had to have his eyes removed the next day because the melanoma was in his eyes.) So polarized sun glasses from an early age, of course high numbered sun block, but also long sleeves even in the summer, and rather than shorts, light weight cotton long slacks. Swim, of course, but use sun block, and sit in the shade and put on a light cotton wrap. The one advantage of a fashion Burka is that it is the right idea to avoid Melanoma. But then the genetic predisposition to Melanoma is not common in the Arab or South Asian world. Remember that water reflects the sun — so even if you are in the shade at the shore, the reflection can get you. Melanoma is not the result of one bad sunburn — it is totally cumulative. The small sunburn at age three or four counts and adds to another one at age 14. We really need a culture that does not value the suntan. We need attractive vacations that do not involve laying out in the sun. These days in Minnesota every 15 minutes the TV ads are about a vacation someplace on a sunny beach or cruise. I would find attractive Northern Norway right now if I wanted to travel. I may have done 37 years, but sunny beaches are still ugly. Greenland in Winter (on SAS flights they call it Northern Denmark). We need more “real” multi-culturalism.
As I said earlier, I’ve spent years looking at the lists of which medical groups have the best outcomes. Jane has one of the very best in the US. For the next months she needs a total partnership with the crew in which she has put her trust — and I know how hard that is — but the next months of healing, and treatment and all — that is what may give her decades more of life. And that is what we are after, isn’t it.

Amen, LL. Your pointed question ties right into Jane’s whole passion with regard to this FDL blog, to boot. Being afraid to “change how we do business” also applies to so much of what’s wrong with Congress and the actions it does and doesn’t take, on so many fronts and in so many battles.
This particular battle happens to be as personal as it gets for Jane and Christy both.
